One register of every delegated task.

A delegated healthcare activity is a clinical task that a registered professional hands to a care worker to carry out for a specific person, such as enteral feeding, rescue medication for epilepsy, catheter or stoma care, or blood glucose monitoring.

Assure records each task against the person it is for, with:

The delegating clinician, such as a district nurse, specialist nurse, dietitian or speech and language therapist.
The delegation date and review date, with overdue reviews flagged.
Protocol notes and a link to the relevant procedure.
How many staff are signed off, so gaps show before they reach the rota.

Competency sign-off

Signed off for that task, for that person.

Training certificates held centrally do not show that a carer can safely carry out a task for one individual. Assure records competency per member of staff, per task, per person.

Assessments record the assessor and their registration, the evidence type (such as observed practice, simulation or a written reflection) and the date reassessment is due.
The competency matrix shows at a glance who is competent, who is expiring, who needs action and who is not assigned.
Expired sign-offs are flagged on the dashboard: staff should not carry out that task until they are reassessed.

What staff see

Staff know exactly what they are signed off for.

On their phone, each carer sees the tasks they are authorised to carry out and the person each one is for, with the review date. It removes the guesswork for relief and bank staff, and gives managers confidence that the right people are doing the right tasks.

Managers can authorise staff, set when each authorisation needs renewing, and revoke it when needed. Every change is recorded in the audit trail.

Common questions

Can we record tasks delegated by our own clinical team?

Yes. The delegating clinician can be an external professional, such as a district nurse, or your own clinical lead, registered nurse or paramedic.

What happens when a competency expires?

It is shown as action needed in the matrix and flagged on the dashboard, with the member of staff, task and person named, until a reassessment is recorded.

Can staff see their own sign-offs?

Yes. Each member of staff sees the tasks they are signed off for, and the person each one is for, on their phone.

We already record training in another system. Do we need both?

Many providers keep mandatory training elsewhere. Assure focuses on what those systems usually miss: task-specific, person-specific competency linked to the delegation itself.
