For case managers, deputies and discharge teamsOpening April 2027

Complex care at home, led by a paramedic.

We work with case managers, deputies, solicitors and NHS and local authority teams to bring complex care home safely. Every package has a named clinical lead, and every carer is trained and signed off for that person before they work alone. Tell us about the person and we'll call you back.

Who we work with

Case managers and deputies

Personal injury and Court of Protection packages, with clear reporting and monthly reviews you can share with the deputy.

Solicitors

Care at home that can be costed and evidenced, with competency records available on request.

NHS teams and social workers

Continuing healthcare, discharge teams and social workers, including fast discharges and local authority spot purchase.

What we take

Adults and children aged 3 and over, at home, wherever home is.

Complex packages

Including tracheostomy (a breathing tube in the neck) and ventilation, brain and spinal injury, PEG and RIG feeding and epilepsy rescue medication.

Children aged 3 and over

Care before and after school and overnight, working with the child's family and health team.

Children's complex care

Fast discharges

Short-term, clinically led support to get someone home from hospital safely, including end of life care.

What you get from us

A clinical assessment

We'll call you back within 1 working day, and carry out the clinical assessment within 3 working days of that call.

A named clinical lead

One clinical lead for each package, and a clear escalation route day and night.

Monthly reviews

A clinical review every month, with a summary you can share.

Competency records on request

Evidence that each carer was trained, observed and signed off for that person's tasks.

Incident reporting

Incidents recorded the same day, reviewed clinically and shared with you, with the duty of candour.

Governance you can check

In real life

Home again, after a life-changing injury

  1. His case manager gets in touch with us.
  2. Our clinical lead meets him and his NHS team at home.
  3. His own small team is trained in his ventilator and tracheostomy care before they start.
  4. Care around the clock, reviewed with him every month.

Life at home, on his terms.

Home, where he wanted to be

  1. His family hopes to bring him home from hospital.
  2. We work with his district nurses, GP and hospice to start quickly.
  3. Gentle visits through the day, and support at night as he needs it.
  4. His family is cared for too, throughout and afterwards.

Comfortable at home, with the people he loves.
