Community support, outreach and enablement

Outreach, enablement and floating support that helps people build skills, keep their home and live the life they want.

Please don't include detailed medical information in a first email. We'll arrange a secure way to share it.

About this service

Community support is about doing, not watching. Our support workers help people plan and do the things that matter to them: getting to work, college or a club, using public transport, shopping and cooking, managing money and staying connected with friends and family.

We also provide tenancy and floating support, helping people settle into a new home and keep it, from paying bills and dealing with letters to keeping on top of the housework and building a routine that works.

Every package has clear goals agreed with the person and the team that funds it. Our records show progress against those goals, shift by shift, and a paramedic leads safeguarding, risk and escalation.

What this looks like

  • Outreach in the community, at the person's pace
  • Enablement: learning to cook, shop, travel and manage money
  • Tenancy and floating support to settle into and keep a home
  • Support with routines, letters, bills and appointments
  • Goals agreed at the start and reviewed with you

Why teams use us

Support you can rely on, led by a clinician, with records you can see.

Clinical leadership

A paramedic leads safeguarding, risk and escalation, and is on call whenever our staff are working.

Records you can rely on

Every shift, incident and concern is recorded digitally, with progress summaries for the people who commission the support.

One provider, children to adults

We support children, young people and adults, from 14 to 25 and beyond, so there is no hand-off at 18.

Safer recruitment

Enhanced DBS with barred list checks on the Update Service, references, and an induction before any shift.

How it works

From referral to the first shift, with everything agreed in writing.

  1. Refer

Tell us about the person, the support and the hours, using the referral form or by email.

  2. Screen

We review every referral the same working day and come back to you with questions or next steps.

  3. Confirm in writing

We agree the support, the staff and the start date with you in writing before anything begins.

  4. Report

You get records of every shift and regular progress summaries, and we flag concerns straight away.

Common questions

What is the difference between outreach and enablement?

Outreach is support out in the community, such as activities, appointments and social time. Enablement focuses on learning skills, so the person needs less support over time. Most packages include some of both.

Do you provide floating support for tenants?

Yes. We help people settle into a new home and keep it: bills, letters, routines, housework and getting to know the area. We work alongside the landlord or housing provider, not instead of them.

Can support be reduced as someone becomes more independent?

Yes, and that is often the goal. We review progress with you and the funder and adjust the hours when the time is right.
