Transition support for young people aged 14 to 25

Steady support for young people moving into adulthood, with one team that stays with them from 14 to 25 and beyond.

About this service

The move from children's to adults' services is one of the hardest times for young people with complex needs and their families. Services change, people change and support can drop away at 18. We support young people through it, with the same small team before and after the move.

Our support workers help young people build the skills and confidence for adult life: travelling independently, managing money, cooking, college, work and relationships, and having a say in their own plans.

Because we support both children and adults, there is no hand-off at 18. A paramedic leads every package and works with the young person, their family and both teams to keep the plan on track.

What this looks like

  • The same team before and after the move to adult services
  • Independent travel, money, cooking and daily living skills
  • Support with college, training, volunteering and work
  • Helping the young person take part in their own plans and reviews
  • Working with children's and adults' teams together

Common questions

At what age can transition support start?

From 14, and it can continue well beyond 25. Starting early gives the young person time to build skills and get to know their team.

Does support stop at 18?

No. We support children and adults, so the same team can carry on when the young person moves to adult services and funding changes.

Can you attend reviews and planning meetings?

Yes. With the young person's agreement, we share progress and attend reviews so everyone has the same picture.
